It costs nothing to take our survey and see if you might qualify for our program! There is no obligation if you do qualify to take any next steps unless YOU want to! The first step is to see if you're a potential candidate. If you qualify then you'll have an opportunity to do a discounted virtual exam with us. From there we will look at a personalized plan for every candidate and the treatment plan costs will vary based on the severity of the person's condition.
What Happens After I Take Your Survey?
After the survey we will reach out to you to let you know if you qualify for a virtual consultation and exam OR if we need to get more information first. My husband and I met with Dr. Zemba and his case manager this morning concerning my neuropathy. Dr. Zemba conducted nerve testing on my feet and then did a back X-ray to find the best treatment for me. They were very thorough going over my health conditions. They made sure to answer any questions we had. They both explained everything in a manner I could understand. He learned how far we came to get there and found a doctor to treat me who is closer to our home. We were very pleased with the experience and would recommend the practice to anyone having back or nerve pain.
